Summary:
Social workers provide psychosocial care in the hospital setting. Social workers link patients with community and agency resources. Social worker engage patients with the necessary social and emotional means for a safe discharge.
Essential Functions:
1. Provides psychosocial support services to patients and families
2. Collaborates with providers, nursing and ancillary staff
3. Connects patients with community resources
4. Engages in practice that comply with regulatory standards, patient safety and performance improvement initiatives.
