Job Description
The High Fidelity Mental Health Wraparound Case Manager engages high-need youth and their families in developing, implementation, and monitoring an intensive, individualized care plan. This includes helping the team develop a natural support system, working on the development of an integrated plan and establishing a strong relationship with the family. This partnership is important as the youth and family work to address challenges and make changes to their lives. The NC Wraparound Facilitator is a person who safeguards that the principles and phases of the High-Fidelity Wraparound (HFW) process are delivered with fidelity to the model.
Responsible for the overall management of the High-Fidelity Wraparound Team by coaching and teaching the wraparound process. Coaching is the process of demonstrating the process, consulting with them on how to use the process, and helping them build the skills necessary to do it well. Coaching will involve hands on teaching (shadowing), then practicing through rehearsals and roleplays, and finally providing ongoing coaching and consultation throughout the credentialing process.
Bachelors degree in human services related field (experience can be substituted). Full-time, dedicated position and must meet requirements as qualified professional with two years of experience providing services for children, youth, and families.

Education: Minimum of a Bachelors degree
Work experience: A minimum of two (2) years of documented clinical experience working with the population served is required.
Knowledge, Skills & Abilities: Ability to lead an entire department and be a positive leader, role model and mentor for all staff. Ability to effectively present information to groups, service users, families & guardians,government officials and/or public. Ability to communicate effectively in verbal and written form. Ability to assess and observe clients to provide appropriate treatment. Ability to relate positively with persons with disabilities, family members, co-workers, community agencies, etc. Ability to follow oral and written instructions. Ability to use computer for work-related tasks. Organizational and communication skills that foster teamwork.
Travel Requirements: Service is delivered in the community so travel will be required.
