Requirements: The ideal candidates must be a graduate of an accredited school of social work. At least 2 years' experience working in social work, which may be in an acute care setting. Must have effective communication skills, knowledgeable of current community services available, be reliable and have a positive attitude, which fosters a cooperative and positive atmosphere.
Knowledge: Must have the clinical knowledge and critical thinking ability to effectively plan and provide coordination of patient care consistent with standards and regulations; must have exceptional interpersonal and customer services skills; must be able to effectively solve unique problems as they arise or identify when to consult the intake supervisor. Must have knowledge of current Medicare, Medi-Cal, State and Federal regulations.
Job Types: Part-time, Per Diem, Contract
Pay Grade: Estimated Annual Compensation: $140K to $195K (position is paid on a per-visit basis)
