Novari Primary Care is a private medical practice that specializes in the treatment of geriatric patients who reside in Adult Family Homes and Assisted Living Facilities. We serve patients in *Washington State*. We are currently seeking an Advanced Registered Nurse Practitioner specializing in the care of homebound *geriatric *patients in the *Skagit and Whatcom County* regions.
Ideal candidates should have a passion for treating the geriatric population and should be well-versed in the common conditions that are presented in this demographic, such as dementia, cardiovascular disease, endocrinological conditions, neurodegenerative diseases, to name a few.
This position involves in-person and telehealth visits to ensure full compliance with Medicare regulations. For this reason, ideal candidates should be located or are willing to travel to *Skagit County and Whatcom County*. Patients are seen on rotation. Visits will include 1-hour intakes and 30-minute follow-ups. We also provide acute visits to patients that are experiencing exacerbations.

*Role Specifics*:
● Prescribe or administer treatment, therapy, medication, and other specialized medical care to treat or prevent illness, disease, or injury as per the process and requirement
● Complete all relevant details in the EMR reports depending upon formats.
● Order and interpret tests and analyzing records, reports, and examination information to diagnose patients' condition.
● Refer patients to medical specialists or other practitioners when necessary.
● Collect, record, and maintain patient information, such as medical history, reports, and examination results through the clinical management system
● Explain procedures and discuss test results or prescribed treatments with patients if required
● Coordinate with nurses and other health care providers.
● Ensure all reports are shared with the concerned teams as per agreed timelines
