BronxWorks is hiring (1) Health Educator for the Positive Living Program

Qualifications (education, professional certifications or licenses, experience, proficiencies)

  • Bachelor's degree in health education or health related field is required
  • Experience facilitating groups is preferred
  • Strong writing and public speaking skills is essential
  • Two years of experience working in the field of HIV/AIDS or other chronic illness
  • Two to three years of experience developing and facilitating health education
  • Possessing an understanding of community-level work and the importance of collaborating and coordination with other organizations
  • Effective communication and documentation skills
  • Cultural and linguistic competence for the population served
  • Follow-up and computer experience are required
  • Bilingual English/Spanish proficiency is preferred
  • Proficiency in Microsoft Office Suite and other standard business applications
  • Essential Duties and Responsibilities
  • Provide information about the HIV/AIDS Case Management and Health Education Services program to clients, BronxWorks programs, and programs in the community
  • Coordinate and facilitate group treatment education
  • Research, prepare and disseminate educational and informational materials relevant to health promotion and disease prevention with focus on Sexually Transmitted Infections/Disease
  • Conduct outreach and recruit clients for intervention programs
  • Help orient clients to this program by introducing them to staff, and reviewing their rights and
  • responsibilities
  • Support Case Manager with development of client assessment and implementation the client’s service plan
  • Provide reminders of health care and other appointments for services to clients
  • Escort clients to appointments, and model self-management and self-advocacy skills
  • Conduct individual Health Education sessions and document the session in a note in the client’s chart that summarizes the client’s participation, progress, accomplishments, further needs and referrals
  • Enter services provided into AIRS
  • Prepare monthly reports about services provided
  • Attend and participate in training/informational sessions that are appropriate for Health Educators.
  • Maintains a calendar of educational and outreach activities to conduct outreach
  • Perform additional duties as assigned

Program Specific Responsibilities

  • Provide reminders of health care and other appointments for services to clients
  • Escort clients to appointments, and model self-management and self-advocacy skills
  • Help in implementing the client’s service plan
  • Meet with supervisor at least monthly
  • Perform additional duties as assigned
  • Conduct outreach to engage clients into services
  • Conduct workshops
  • Carry a small case load

Skills/Abilities

  • Strong communication and presentation skills with the ability to represent the agency at functions and meetings
  • Ability to initiate and develop communication in the community; knowledge of the Bronx and its communities
  • Ability to use a computer for prolonged periods
  • Ability to occasionally lift and/or move up to 10 pounds
  • Ability to stand, walk, or sit for long periods of time
  • Ability to climb five flights of stairs
  • Ability to bend and retrieve objects and/or documents
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather
