What To Expect (Job Responsibilities)

• Manage and process new referrals for patient care, ensuring timely admissions
• Maintain clear communication with patients, families, and referral sources through phone interactions
• Collaborate with hospitals, physicians, and other healthcare providers for seamless patient care
• Enter referral information into the Electronic Medical Records (EMR) system accurately
• Promote excellent customer service in all interactions with patients and staff

What Is Required (Qualifications)

• Graduate of an accredited school of Vocational Nursing
• Unencumbered LVN-License Vocational Nurse license for the state of residence
• Must obtain an LVN-License Vocational Nurse of California within 3 months of hire
• Minimum of 1 year of recent relevant experience
• Ability to manage multiple tasks in a fast-paced work environment
