Transitions LifeCare, a leader in hospice, and palliative care, is hiring immediately for a Weekend On-Call LPN – Part-Time position. As part of our compassionate team, you will play a vital role in providing exceptional care and support to patients navigating end-of-life or life-changing illness.

This position offers a sign-on bonus and is ideal for those looking to contribute to a dynamic healthcare team while providing flexible weekend hours.

The schedule for this position is Saturday/Sunday 9:00am-9:00pm (hours can be flexible).

Responsibilities:

Patient Follow-Up: Return or make follow-up phone calls to patients as directed by the Triage RN.
Assessment & Recommendations: Under the guidance of the Triage RN, make recommendations related to patient assessments, ensuring adherence to policies and procedures.
Collaborate with Physicians: Communicate any changes in patient status or needs with attending physicians or TL on-call physicians.
Patient & Caregiver Education: Instruct patients and caregivers on any changes to the patient’s Plan of Care.
Coordinate Additional Care: Assign visits to RN, Social Worker, or Chaplain when necessary, in collaboration with the Triage RN.

Requirements:

Licensure: Current LPN license in North Carolina (BSN preferred).
Experience: Previous experience in hospice, home health, or palliative care is a plus.
Transportation: Must have reliable transportation to visit patients in the service area.
Compliance: Must pass all pre-employment and annual tests and meet state and federal guidelines.
