Details:

We are looking for a part-time LPN to work 8-hour shifts Friday, Saturday, and Sunday from 12:00 a.m. to 8:00 a.m. This role will provide backup on-call support to our on-call nursing team. This position requires travel across the service area to healthcare facilities, assisted living homes, patients’ homes, and home office when needed. The ideal candidate is calm under pressure, confident in decision-making, and comfortable working independently while collaborating with a broader interdisciplinary team.

Key Responsibilities:

Triaging after-hours and weekend hospice calls to ensure timely and appropriate interventions
Supporting the intake and referral process with compassion and efficiency
Completing in-person field visits when necessary to assess patient needs
Accurately documenting in the EMR system and communicating with the care team
Providing holistic, patient-centered care with sensitivity to emotional, physical, and spiritual needs
Qualifications:
Current LPN license in PA
Hospice experience required
Experience with EMR systems required
Proficient with computers and comfortable navigating digital tools
Strong interpersonal and communication skills
Ability to work independently and prioritize tasks effectively
Reliable transportation, valid driver’s license, & auto insurance for travel within the service area
