Job Details

• RN Medical/Surgical position at Cleveland Medical Center, University Hospitals, Ohio.

Job Requirements

• BLS - Required for clinical positions.
• ACLS - Optional, but preferred for clinical roles.
• License/Certificate Verification - Primary Source Verification of professional license and/or certification, as required for the position ran within 30 days of start updated upon expiration.
• COVID-19 Vaccine - Proof of vaccine series or completion and approval of client medical or religious exemption form. Preferred, not required.
• MMR - Positive MMR titers ONLY from within the last 15 years.
• Fit Test - Completed within 1 year of start updated annually.
• Hepatitis B - Proof of vaccination or signed declination.
• Positive PPD- Negative Chest X-Ray and Annual TB Questionnaire - If positive, CXR within 5 years and current Questionnaire.
• Varicella - Positive Varicella titers ONLY from within the last 15 years.
• Tdap - Proof of vaccination within past 10 years, declinations not accepted.
• Annual Negative PPD - If 2 step PPD not done within past year.
• UHS I-9 Attestation - Agency to provide completed I-9 or attestation that completed I-9 is on file and candidate is eligible for work.
• UHS Social Security Trace - If not ran with background check, to be complete within 1 year of start.
• UHS NSO - If not ran with background check, to be complete within 30 days of start.
• UHS Ohio Fingerprints - Live in Ohio for 5 years: Ohio Fingerprints (BCI&I check) AND FCRA-Compliant National Database Criminal background check.
• UHS Education Verification - Highest level of education verified.
• UHS Employment Verification - Must provide verification of employment within 5 years.
• UHS Skills Checklist - Clinical Only: Skill specific required to be complete within 1 year prior to start.
• UHS Competency Exam - Competency Exams for the relevant department. Must score at least 80%.
• UHS Core Competencies - 80% passing score required, Completed within 1 year prior to start.
• Physical/Health Statement - Physical Exam completed within one year of initial start date.
• Influenza Vaccine - Proof of vaccine from Oct-March or exemptions must be submitted for approval.
• 2 Step PPD OR IGRA (Quantiferon/Tspot) - 2-Step TB test with negative reads within the last 6 months.

Additional Details

• Contract Length: 13 weeks.
