\nThe care provided by our Hospice McCall team allows people to pass with dignity in their own homes, in familiar surroundings, with those they love by their side. The RN Case Manager/Coordinator works with physicians and multidisciplinary team members to develop and manage a plan of care for an assigned case load. This is a direct care position providing in home care to include symptom management, emotional and spiritual support, medication management, education and IDT oversight. The person needs to be able to work autonomously and as a team, have interpersonal and communication skills, and have a true passion and desire to care for end-of-life patients to ensure their needs are met.
