Summary Conducts health assessment interview focused on wound, ostomy, and continence related care-performing a focused physical assessment. Collects data and information relative to the health or situation of the resident with wound, ostomy, and/or continence care needs. Analyzes assessment data to determine actual or potential problems/issues related to wound, ostomy, and/or continence care needs. Develop and Implement plans for wounds, ostomy, and/or continence care that are focused on strategies to attain expected, measurable outcomes. Reviews existing orders and makes recommendations to physicians, managers, and other members of the healthcare team that are focused on healing, maintaining, or supporting the residents wound care goals. Complete weekly wound roads with wound physician and document on wounds weekly using Ekare system. When appropriate-teaches, consuls, and assists families and residents to assume responsibility for prevention of complications as they relate to patients with wound, ostomy, and continence problems. Develop, provide, and evaluate staff education programs, orientation, in-services and continuing education as it relates to wound, ostomy, and continence. May be assigned other duties as appropriate.
