Role Overview:
Join our dynamic home health care team as a Registered Nurse (RN) Case Manager in Fort Collins, CO. In this role, you will collaborate closely with families to develop personalized care plans and deliver skilled nursing services in-home.

Responsibilities:

Conduct in-home visits every 60 days to assess patient needs and progress.
Develop comprehensive care plans and provide hands-on care to children with IDD.
Educate and support parents, offering coaching and guidance on their child's health.
Utilize telehealth tools to enhance communication with the care team and families.
Advocate for children's health and contribute to our mission of achieving superior health outcomes.
Requirements:

Active RN license in Colorado or a compact state, with a minimum of 2 years of nursing experience.
Experience in home care and familiarity with pediatric care.
CPR, BLS, and preferably PALS certifications.
Ability to thrive in a fast-paced startup environment.
Must reside in Fort Collins, CO, or be willing to relocate.

Daily Schedule:

Monday: Mandatory nurse meeting in Denver office from 9:00 am to 10:30 am; rest of the day dedicated to remote administrative tasks.
Tuesday to Friday: Conduct in-person recertifications and start-of-care assessments as scheduled, with remaining time allocated for remote charting and family communication.

Pay: $35.00 - $40.00 per hour
