The Role:
Registered Nurse Case Manager - Home Health Care (RN)
Location: Fort Collins, CO (70% in person, 30% remote)
We are seeking a dedicated and compassionate Registered Nurse to join our home health care team as an RN Case Manager. In this role, you will collaborate closely with families of children with intellectual and developmental disabilities (IDD), providing high-quality, skilled nursing care and developing personalized care plans.
Responsibilities:
Conduct home visits every 60 days to assess patient needs and progress.
Develop and implement comprehensive care plans, offering in-home nursing care and supporting families.
Educate and empower parents about their child's health, providing coaching and guidance.
Utilize telehealth tools effectively to enhance care delivery and communication with the care team and families.
Advocate for children's health outcomes and contribute to our mission of redefining pediatric care.
Requirements:
Registered Nurse with at least 2 years of experience, licensed in the state of Colorado or holding a compact license.
Experience in home care, with a preference for those who have trained family CNAs.
Comfortable in a fast-paced startup environment and willing to work from home.
CPR certification required; BLS and PALS certifications preferred.
Pediatric nursing experience strongly preferred.

Daily Schedule:
Monday:
Mandatory Nurse meeting in Denver office from 9am to 10:30am; rest of day for remote administrative tasks.
Tuesday-Friday:
In-person recertifications and start of care assessments as scheduled, with remaining time for remote charting and family communication.
