JOB DESCRIPTION SUMMARY
We are seeking a compassionate and experienced Registered Nurse Case Manager to join our team. In this role, you will plan, organize, and direct hospice care services, leveraging community resources to meet the needs of individuals and families in their homes and communities.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care
Conduct initial, comprehensive, and ongoing assessments to determine hospice care needs.
Utilize the nursing process to provide professional care, including physical assessments and history documentation.
Develop and implement individualized care plans in collaboration with patients and families, focusing on palliative nursing actions.
Administer medications and treatments as prescribed by physicians.
Counsel patients and families on nursing and related needs, fostering self-sufficiency and comfort care.
Coordinate patient care as the Case Manager for assigned caseloads.
Communication
Maintain accurate clinical notes and communicate changes in patient condition to physicians.
Coordinate care plans with community health professionals.
Educate patients and families on self-care techniques and health management.
Collaborate with interdisciplinary teams to ensure comprehensive patient care.
Attend interdisciplinary group meetings to optimize patient support and emotional well-being.
Additional Duties
Participate in on-call duties as per policy.
Ensure availability of necessary equipment and services.
Supervise ancillary personnel and delegate responsibilities as needed.
Pursue ongoing professional development through education and training.
Actively engage in quality assessment and performance improvement initiatives.
POSITION QUALIFICATIONS
Current licensure as a Registered Nurse in the state of California.
Graduate of a National League for Nursing accredited school of nursing.
Valid CPR certification.
Licensed driver with a reliable, insured automobile.
Minimum one year of recent professional nursing experience; hospice or home care experience preferred.
Strong organizational skills with the ability to work independently.
Excellent communication and problem-solving abilities.
Physical capability to perform required nursing tasks.
