JOB DESCRIPTION SUMMARY
The Registered Nurse Case Manager plays a pivotal role in planning, organizing, and directing hospice care, leveraging nursing expertise with a focus on community health education. This position involves working closely with individuals and families within their homes and communities to ensure comprehensive care.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care:
Conducts thorough initial and ongoing assessments of patients and families to determine hospice needs, including comprehensive physical assessments and histories.
Provides compassionate nursing care by utilizing the nursing process and continually assessing and evaluating patient and family needs.
Develops personalized care plans based on nursing diagnoses and incorporates palliative nursing actions, ensuring active participation from patients and families.
Administers prescribed medications and treatments, and counsels patients and families on health-related needs.
Facilitates daily living activities and supports patients in achieving self-sufficiency and optimal comfort care.
Acts as a Case Manager to coordinate care for assigned caseloads.
Communication:
Maintains accurate clinical documentation and communicates effectively with physicians regarding patient needs and condition changes.
Coordinates care plans with community health professionals and educates patients and caregivers on self-care techniques and health management.
Collaborates with interdisciplinary teams to ensure comprehensive patient care and attends regular group meetings.
Provides a safe environment for patients and supports continuity of care for families and team members.
Additional Duties:
Participates in on-call duties as per policy and ensures availability of necessary equipment and services.
Supervises ancillary personnel and delegates responsibilities when necessary.
Pursues personal and professional development through continuing education and participation in quality assessment and improvement activities.
Assumes responsibilities for assigned case assignments as required.
POSITION QUALIFICATIONS:
Registered Nurse with current licensure in the state of California.
Graduate of a National League for Nursing accredited school of nursing.
Current CPR certification.
Must possess a valid driver's license and have access to a reliable, insured vehicle.
Minimum of one year of professional nursing experience within the last three years, or a baccalaureate degree in nursing from an accredited program and a current RN license.
Preferred minimum of two years' experience, including public health, home care, or hospice nursing.
Self-directed with the ability to work independently and prioritize workload effectively.
Strong observational, problem-solving, verbal, and written communication skills.
Demonstrates physical abilities necessary for the role, including prolonged standing, walking, and lifting patients and equipment.
