Job Description

The Registered Nurse Case Manager coordinates and delivers hospice care with a focus on community health education and patient-centered support. This role involves assessing patient needs, developing care plans, and ensuring comprehensive nursing care within homes and communities.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Patient Care:

Conducts thorough initial and ongoing assessments of patients and families to determine hospice care requirements.
Provides compassionate nursing care utilizing the nursing process, including assessment, planning, implementation, and evaluation.
Collaborates with patients and families to establish care goals based on nursing diagnoses, integrating palliative nursing interventions.
Administers medications and treatments as prescribed, ensuring adherence to physicians' care plans.
Educates patients and families on health care strategies, promoting self-care and comfort.
Coordinates patient care as a Case Manager, ensuring continuity and effectiveness.
Communication:

Maintains accurate clinical documentation, including patient condition and care details.
Collaborates with physicians to address patient needs and report changes in condition.
Coordinates care plans with community health providers and educates patients on self-care techniques.
Participates actively in interdisciplinary team meetings to optimize patient care and support.
Additional Duties:

Participates in on-call duties per policy requirements.
Coordinates equipment and services essential for patient care.
Supervises ancillary staff and delegates responsibilities as needed.
Engages in professional development through continuing education and training.
Contributes to quality assessment and performance improvement initiatives.
POSITION QUALIFICATIONS:

Current licensure as a Registered Nurse in the state of [State].
Graduate of an accredited school of nursing.
Maintains current CPR certification.
Valid driver's license and insured automobile in good working condition.
Minimum of one year of recent nursing experience; experience in public health, home care, or hospice nursing preferred.
Self-directed with strong organizational and communication skills.

Capable of managing workload independently and effectively.
PHYSICAL DEMANDS:
