Momentum Healthcare Staffing is seeking a Family Medicine or Internal Medicine Physician to provide tele-health evaluations to refer patients for Home Health Visits located in the Pasadena area, CA.The physical location is in the Pasadena area of CA; however, the requirement is to have an active CA license.Please see the details below to apply! Requirements: Specialty: Family Medicine or Internal Medicine Education: MD/DOState License: CaliforniaExperience: Home health experience is preferredPatient Population: Hospice, mostly older adults Details of the Opportunity:The first step toward receiving home health care is to obtain a physician's orders and work with a home health care company to develop a detailed care plan.The physician will provide a complete initial consultation with the patient, during which they assess the patient's needs and develop a plan for treatment.Once the patient has been referred, the home health care staff will implement the plan, following all physician orders and keeping the physician updated about the patient's progress. Schedule:This need currently is approximately 8 hours per month and can be scheduled around the providers current workload.Hourly/Salary:$120 per hour - Telehealth.
