Responsibilities
Heritage Home Health Physical Therapists (PTs) work with patients, their families, and other healthcare professionals in the patients place of residence. In addition to performing visits and completing coordination of care, the PT is the point of contact for all disciplines involved with providing care to the patients. The PT follows agency, state, and federal policies and procedures. The PT will consult with the patients primary care physician and attend meetings as required.
