Patient Pre-Registration - at least 1 day in advance • Referral shells are linked to visits:

Attach insurance referrals or auth number (when applicable) o New patients o UHC Comm plans and Any HMO insurance that require electronic referral. o Injections/Procedure o External Surgeries • Daily work queues call (as assigned) • ALL forms must be the most up to date in all locations. • Confirm all appointments, at least 1 day in advance per location

Upon checking IN patients: Always Acknowledge Patient and give proper greeting. • Verify Phone Number and Address is correct. • Photo ID scanned (most updated) o a patient cannot be seen without proper photo ID o Input Expiration Date • Insurance Card scanned. o Most Updated and Linked coverage on EPIC • Pharmacy • Emergency Contacts • HIPAA and Financial E-Form signatures (done every year) • Consent to Treat E-form signatures •'

My CHART sign up is completed. o Input proper communications preferences. • Referrals information: scanned to EMR. o Confirm EPIC Shell is linked to appropriate visits. o Input insurance referrals or authorization numbers to Epic Shell o Scan referral/auth information hard copy into media or snip into referral shell ***COLLECT TOS COPAY***

Front Desk Staff: Check out: ? Re-Confirm that all the above has been done, if not please finish the process. ? Make sure patient is given a f/u appointment. o notate appt note with reason if not scheduled example-“pt stated will call to schedule” ?
