Job Description:
The social worker will complete a comprehensive and culturally competent assessment of the client"s psychosocial, environmental and financial needs. Which includes appropriately adding SW pathway and involving client, family and other health care practitioners in assessment. Develops goal plans that incorporate assessment data, identify expected outcomes and enhance the clients" ability to manage their disease process or disability. Develop individualized pathways and revise based on pt/cg needs, resources, finances and psychosocial deficits. Ensures patient is scheduled out correctly using correct codes. Communicates appropriate data to other persons involved in the individual"s or family"s care such as other team members and/or other health providers. Social worker will develop goal plans that incorporate assessment data, identify expected outcomes and enhance the clients" ability to manage their disease process or disability. Social worker utilizes appropriate intervention strategies to assist the home health client to meet their goals. Adapt intervention strategies to work effectively with clients from different age groups, cultures, socioeconomic backgrounds and differing states of mental health and disability. Documentation will be done in the home or prior to the next patient. Correct template will be used to document coordination notes. Orders are done correctly including spelling, description, when to send or not to send, correct scheduling codes, etc.). Patients are prepared for discharge in accordance with the NASW Code of Ethics and Cornerstone VNA discharge process. The social worker will demonstrate knowledge of the available community resources which may be engaged on behalf of any client of the home health agency. Establish and maintain professional relationships within the community. Advocate and educate clients and families, as appropriate, to access resources and implement services. System of providers is accessed through network and data base resources which is maintained and updated by the social work department. The social worker evaluates his or her own social work practice in relation to professional practice standards. The social worker engages in performance self-appraisal on a regular basis, seeks constructive feedback from peers and management, identifying areas of strength as well as areas for professional development. Effective preceptor to new employees. Participates in educational activities in-house and office to maintain NASW licensing. Actively participates in the agencies Quality Improvement Performance Improvement (QAPI) plan and projects. Seeks opportunities in the agency and/or community to provide education or improvement in care initiatives. The social worker is responsible for adhering to agency policy and guidelines concerning completion of time logs accurately, tact, attitude, punctuality, attendance, discretion, dependability, and productivity.

Ability to represent Cornerstone VNA in a trusted and professional manner
Strong communication skills

Excellent critical thinking and organizational skills
Valid driver"s license in good standing and vehicle insurance required
