Job Description

 

The Hospice Medical Social Worker plays a critical role within our compassionate team, focusing on improving the quality of life for patients and their families facing terminal illnesses. This position entails providing emotional support, counseling, and guidance to both patients and their loved ones during one of the most challenging times in their lives. As a key member of the interdisciplinary hospice care team, the Medical Social Worker will assess the psychosocial needs of patients and families, collaborate with healthcare professionals to create personalized care plans, and facilitate access to community resources. Furthermore, the Medical Social Worker will engage in bereavement support, addressing the emotional, financial, and social aspects of end-of-life care. The ideal candidate will possess a strong understanding of the hospice philosophy, demonstrating empathy and compassion, as well as the ability to contribute to a supportive and healing environment. With a commitment to holistic care, the Medical Social Worker will serve as an advocate for patients, ensuring their dignity and comfort are prioritized throughout the hospice journey. This position requires not only clinical expertise but also the sensitivity to navigate complex emotional landscapes, making it essential for candidates to come equipped with strong interpersonal and communication skills.

 

  • They work closely with medical teams to coordinate treatments, services, and resources that can help improve a patient’s quality of life. 
  • They provide a wide range of supports, based on the specific needs of the patient and the patient's family. They advocate, problem solve, listen, and connect people to resources.
  • They also provide resources such as counseling and referral services.
  • They help families cope with the stress of dealing with a terminal illness and coordinate care for those who may be facing financial or other hardships due to the illness.

 

 

Qualifications:

  • Master's Degree in Social Work
  • Must have experience as a social worker
  • Must have reliable transportation
