The Hospice Social Worker provides direct casework and counseling services to the patient/family unit. The Social Worker will provide assessments of psychosocial problems and define goals and interventions to provide excellent end of life care.


The Hospice Social Worker:

  • Is a member of an interdisciplinary team (IDT) and provides social services in accordance with the interdisciplinary plan of treatment.
  • Participates in the preparation, evaluation, and execution of the interdisciplinary plan of care to assure that the highest quality of care is given to those served.
  • Recognizes and utilizes community and family resources to assist the patient and family with end-of-life care needs.
  • Develops the plan of care with the IDT to deal with personal, financial, and environmental difficulties that may interfere with care.
  • Assists with funeral planning as needed.
  • Keeps abreast of social work trends and knowledge.
  • Participates in education both internally and externally to promote professional growth and understanding of hospice.
  • Maintains continuity of patient care by liaising with other health care professionals.
  • Attends IDT conferences and care plan meetings.
  • Arranges for extended care placement or respite placement when necessary.

 

Job Requirements

  • Must have reliable transportation and a valid driver’s license.
  • Ability to manage your schedule effectively and efficiently.
  • Minimum of one year in social work, preferably in a healthcare related setting.
  • Must possess a Master of Social Work (MSW) from a school of social work accredited by the Council on Social Work.
  • Must be licensed by the State of Indiana.
