Admissions Screener

LPN Preferred

Pay Rate: $26.00/hr to $36.00/hr depending on license and experience.

The McGuire Group has an exciting opportunity for a Full-Time Admissions Screener to join our caring and knowledgeable team supporting our facilities in Erie County. The admissions coordinator coordinates all admissions and transfers to the facility, working closely with the nursing staff and social services relating to the prospective resident’s needs.

Responsibilities

• Responsible for screening and recommending patients for long-term or subacute care
• Visit designated hospital assignments daily
• Knowledge of age-specific developmental factors specific to adult and geriatric residents (i.e., physical, cognitive, and socialization factors) in carrying out the admissions process
• Coordinates room changes and inter-facility transfers upon request and following approval of resident’s physician
• Interacts with residents and family members, co-workers, and clinical and ancillary staff in a non-judgmental, supportive, and calm manner
• Maintains a daily census record for the assigned facility
• Additional tasks as assigned

Qualifications

• Bachelor's degree in Social Work, Nursing, Therapy, or another health-related field from an accredited institution
• Experience in Admissions Screening and long-term care is preferred
• Must be flexible, professional, and reliable
• Strong customer service skills
• Must possess a valid New York State Driver’s License and have transportation available for job-related travel
