Schedule: Full Time, 40 hours per week

\n \n\n \n

Shift : Primarily Monday - Friday, 8am to 5pm, rotating weekends

\n \n\n \n

Location : Team Pink: This position will be conducting patient home visits in the East Orlando, South Sanford, Winter Springs, Oviedo, East Longwood, Waterford, Christmas area which includes but is not limited to the following zip codes: 32773, 32732, 32750, 32708, 32765, 32766, 32826, 32820, 32709, 32828, 32833, 32831, 32832

The role you’ll contribute: Provides vocational nursing services under the supervision of an RN, to patients in their homes.

\n \n\n \n

• Performs nursing skills safely and competently.

• Performs patient data collection in a holistic manner.
\n \n
• Provides accurate and complete documentation.
\n \n
• Schedules daily visits to facilitate patient needs.

• Plans patient care in consultation with Registered nurse.
\n \n
• Assists with patient/caregiver teaching.
\n \n
• Demonstrates flexibility in response to unexpected changes and the ability to adjust priorities and use resources.
\n \n
• Maintains open communication with RN Case Coordinator and RN Supervisor.
\n \n
• Demonstrates suitable time management skills.
\n \n
• Meets agency's expectations regarding productivity.
\n \n
• Participates in monthly case conferences
\n \n
• Participates in agency Performance Improvement activities.
\n \n
• Demonstrates good time management skills.
\n \n
• On call, as requested, RN as back up.
\n \n
• Provides orientation to newly hired LPNs and hosts student LPNs.
\n \n\n \n\n

The expertise and experiences you’ll need to succeed :

\n \n\n \n

Minimum qualifications :

\n \n\n \n\n

\n
• Licensed Practical Nurse or Licensed Vocational Nurse
\n \n
• BLS Certificate
\n \n
• Valid Driver's License and Auto Insurance
\n \n
• One year of medical/surgical experience
\n \n\n \n\n

\n

Preferred qualifications:

\n \n\n \n\n

\n
• One year experience in community health nursing
\n \n
• Thirty (30) hours of Florida Board approved IV Therapy Course
\n \n\n \n\n
