The State of New Hampshire, Department of Health and Human Services, Division of Public Health Services has a full time vacancy for Health Promotion Specialist. This position is subject to renew every two years.
SUMMARY:
Plans, organizes, coordinates, implements and evaluates delivery of evidence-based chronic disease prevention and management programs.
RESPONSIBILITIES:
Reviews, modifies, and implements strategies, activities, policies and procedures related to the delivery of evidence-based chronic disease prevention and management programs services to achieve Section goals and objectives.
Designs and presents marketing and communications materials and tools to promote evidence-based chronic disease prevention and management programs, in conjunction with chronic disease and public health program staff, other government agencies, and officials.
Evaluates program objectives, activities and grant deliverables related to the delivery of chronic disease program services and assists program managers to achieve short- and long-term program goals.
Collects, analyzes and reports data to improve chronic disease prevention and management program performance.
Provides ongoing consultation, training and technical assistance to public health, medical and community agencies related to increasing enrollment in evidence-based chronic disease prevention and management programs.
Evaluates the development and implementation of chronic disease program delivery to inform and make recommendations to Program Administrators and Managers about grant strategies and activities.
MINIMUM QUALIFICATIONS:
Education: Master's degree from a recognized college or university with major study in public health education, social work, education, public health, genetics, biology or related biological science. Each additional year of approved formal education may be substituted for one year of required work experience.
Experience: Three years' experience required in developing educational and promotional programs for community organizations, in the area of public health, or in a medical genetics center involving administration, planning, consultation or direct services. Each additional year of approved work experience may be substituted for one year of required formal education at the graduate level only
License/Certification: Valid New Hampshire driver's license and/or access to transportation for statewide travel.
