Pay Rate : The pay range for this role is generally $41,600-$46,600 annually, commensurate with relevant experience and qualifications and in alignment with internal equity. Community Health Action of Staten Island (CHASI) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.

POSITION SUMMARY :

The Health Care Coordination Department provides access to health care services for persons living with chronic illnesses including but not limited to HIV/ AIDS, mental health, and substance use disorders. The Engagement and Supportive Services (ESS) program’s Health Educator provides information and education to persons living with HIV with the focus of achieving optimal treatment adherence, viral suppression, retention in care, and reduced HIV-related mortality. Program members include persons who are not Medicaid eligible, immigrants, persons newly diagnosed, and those who have fallen out of, or are sporadically engaged in HIV care and treatment. Education is offered one-on-one or in group settings. Services are provided in Spanish/English.

DUTIES & RESPONSIBILITIES :

  • Plan, organize, and facilitate Health Education groups including the development of the content, materials, and activities.
  • Provide team-based learning opportunities on the Health Education Curriculum and/or relevant health-related topics.
  • Conduct individual health education sessions for members as they relate to their health care and treatment.
  • Identify eligible clients by sustaining relationships with internal and external service providers through outreach and recruitment activities.
  • Participate in program enrollment activities including the completion of documentation such as Intake/ Assessment, Health Education Screening, and Service Planning
  • Identify strengths and challenges for retention in care, adherence, and viral suppression through the assessment process.
  • Provide information about maintaining and increasing health and wellness.
  • Address barriers to enable client access to all necessary components of health care, including mental health and substance abuse services.
  • Negotiate and advocate on behalf of clients for services that support self-sufficiency and self-management.
  • Collaborate with the Case Manager and Community Health Worker to identify appropriate health education services for clients and program needs.
  • Achieve training requirements and participate in learning opportunities.
  • Complete administrative tasks such as maintaining MetroCard and incentive logs.
  • Adhere to standards for documentation requirements, data submission, and client charts.
  • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values
  • Other duties as assigned.

Requirements:

QUALIFICATIONS :

  • Two (2) years of experience working in the field of HIV/AIDS, behavioral health, substance abuse, health education, or other chronic illnesses REQUIRED
  • High school or equivalent with related experience and strong socio-cultural identification with the target population REQUIRED some college, Associate, or Bachelor's degree PREFERRED
  • Fluency in Spanish & English REQUIRED

  • Knowledge and understanding of care management principles, documentation, referral processes, and follow up.
  • Culturally responsive to individuals living with HIV/AIDS and those at-risk for becoming infected with HIV, especially in immigrant populations.
