Saint Thomas Community Health Center is a Federally Qualified Health Center dedicated to providing the highest quality healthcare to any patient regardless of insurance status or ability to pay. St. Thomas Health Coaches are certified on the job with motivational interviewing training.

Every day patients come into our clinic overwhelmed with navigating complicated healthcare systems, and chronic diseases. The Health Coach comes in to improve a patient's experience at Saint Thomas, and in the best scenarios, arm them with tools to make positive life changes. Health Coaches are afforded the opportunity to work closely with providers, nurses, medical assistants and case management. The role includes developing relationships with patients, reviewing care plans, and setting goals. The health coach offers supportive guidance to help patients foster healthier lives. We are looking for candidates that are compassionate, good listeners, kind, and skilled at problem solving. Health Coaches are an incredible asset and respected member of the medical team here at Saint Thomas. We are dedicated to providing easy access to care. We welcome applicants with bachelor degrees in social work, public health and health sciences, and applicants that hold Medical Assistant or Nursing Assistant licensing.

Essential Job Duties

  • Meet with patients to promote health goals
  • Review social determinants of health
  • Enroll patients into Chronic Care Management
  • Discuss care plans with patients by phone and in person
  • Utilize Motivational Interviewing principles
  • Assist to break down barriers for access to healthcare
  • Take direction from nurses and providers
  • Work and document in EPIC electronic health record system
