Are you ready to make a difference in the lives of patients and their families? We are looking for a passionate Clinical Liaison to join our team at Providence Home Services . This part-time role (28 hours/week) will have you floating between PPMC and PSVMC , where your strong communication and interpersonal skills will shine!
As a Clinical Liaison, you will be the vital link between our home care services and contracted facilities, coordinating essential services such as home health, home infusion, and home medical equipment. Your expertise will help ensure a smooth transition for patients following hospitalization, making a real impact on their care journey.

This position is on-site at our Halsey Building in East Portland, where you will be part of a dynamic team within the Providence Home and Community Care service line. Together, we serve nearly 200,000 people each year, providing a full range of services for individuals facing chronic or life-limiting illnesses.
Pay Range: $50.21 - $70.04 (base pay range; additional compensation may be available)
