Join Our Team as a Clinical Liaison!

Are you ready to make a difference in the lives of patients and their families? We are looking for a passionate Clinical Liaison to join our team at Providence Home Services . This part-time position (28 hours/week) will have you floating between PPMC and PSVMC , where your skills will shine in a supportive environment.

Your Role

As a Clinical Liaison, you will be the vital link between our home care services and contracted facilities. Your strong communication and interpersonal skills will help you:

• Coordinate and arrange home care services, including home health, home infusion, and home medical equipment.
• Educate hospital staff about the services we offer, contributing to patient care and discharge plans.
• Ensure a smooth transition for patients and their families following hospitalization.

What We’re Looking For

Required Qualifications:

• Oregon Registered Nurse License (upon hire)
• National Provider BLS - American Heart Association (upon hire)
• Criminal background check compliance (upon hire)

Preferred Qualifications:

• Bachelor's Degree in Nursing
• 1 year of full-time experience in home health or hospice
• 1 year of full-time experience in home infusion
• 1 year of full-time experience in discharge planning or care management
• Experience in home care with strong customer service skills

Working Conditions

This position is on-site at our Halsey Bldg-Home Service East Portland location. You will be part of a dynamic team that serves our community with compassion and dedication.

Pay Range: $50.21 - $70.04 (base pay range; additional compensation may be available)
