Join Our Team as a Clinical Liaison RN!

This role is all about communication and interpersonal skills , as you will coordinate and arrange essential home care services, including home health, home infusion, and home medical equipment.

Your Responsibilities

• Coordinate home care services between Providence Home Services and contracted facilities.
• Manage clinical referrals, ensuring seamless transitions for patients.
• Collaborate with healthcare professionals to deliver top-notch care.

What We’re Looking For

Required Qualifications:

• National Provider BLS certification from the American Heart Association (upon hire).
• Active nursing license in the states where care is delivered (upon hire).
• At least 1 year of full-time experience in a clinical role.

Preferred Qualifications:

• Bachelor's Degree in Nursing.
• Experience in Home Health, Hospice, or Care Management.
• Background in the healthcare industry.
• Documented success in sales account medical management.
• Familiarity with Epic EMR.

Working Conditions

This is a part-time position with day shifts, located on-site at our Halsey Building in East Portland. The pay range for this role is $50.21 - $70.04 , with additional compensation opportunities available.
