POSITION TITLE: Case Manager - Community Based Services

REPORTS TO: Community Based Services Supervisor

DIRECT REPORTS: None

FLSA STATUS: Non-exempt; regular full-time; employment-at-will position.

SCHEDULE: The work schedule will generally conform to that of a full-time Monday-Friday 40-hour-per-week schedule to be performed within the Center’s hours of operation.

PURPOSE: The CBS (Community Based Services) Case Manager will be responsible for providing comprehensive face-to-face case management services to youth within the home, school, or community settings. The Case Manager will work collaboratively with a multidisciplinary team to support the mental health and well-being of youth clients, ensuring they receive appropriate services and resources to address their wellbeing. This role involves planning, coordination, monitoring, and advocacy to help clients achieve their treatment goals and improve their quality of life.

ESSENTIAL JOB FUNCTIONS:

  1. Provide direct services in clients' homes, schools, and community settings.
  2. Develop and implement individualized service plans in collaboration with clients and their families.
  3. Advocate for clients' needs, assisting them in accessing community resources and services.
  4. Connect clients to crisis intervention services and assist in safety planning alongside the crisis team as needed.
  5. Support clients in developing and enhancing life skills, social skills, and coping strategies.
  6. Work closely with community partners, healthcare providers, and educators.
  7. Participate in treatment team meetings to discuss client progress and adjust service plans as scheduled.
  8. Coordinate with other internal and external service providers to ensure comprehensive care.
  9. Maintain professional relationships with all partners and agencies involved in clients' care.
  10. Complete all required client service documentation accurately and promptly.
  11. Maintain client confidentiality and adhere to all ethical and legal standards.
  12. Document all services provided, including client interactions, referrals, and follow-up activities.
  1. Attend and participate in all required training sessions and professional development activities.
  2. Stay informed about best practices and advancements in mental health and substance use disorder treatment.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES:

  1. Bachelor’s degree in human services preferred or with other significant relevant human services experience. Alternatively, four years of education/experience in the human services field.
  2. Must be at least 21 years of age; Proficient in the English language; Able to pass pre-employment background and drug screening; Have completed or is able to complete within the first 60 days of employment case management and crisis training as required by the State of Kansas.
  3. Experience working with individuals with Serious Mental Illness (SMI), Serious Emotional Disturbance (SED), or Substance Use Disorders (SUD) is preferred.
  4. Strong organizational and time management skills.
  5. Excellent verbal and written communication skills.
  6. Ability to work independently and as part of a team.
  7. Proficient in the use of computers and electronic health record systems.
  8. Required to maintain professional boundaries and confidentiality.
  9. Individuals must be physically capable of operating a company vehicle safely, possess a valid driver’s license, and have an acceptable driving record.

PHYSICAL DEMANDS:

  1. Remaining stationery, such as standing or sitting, for prolonged periods.
  2. Frequently moving from one worksite to another to accomplish tasks.
  3. Adjusting or moving objects up to 10 pounds in any direction.
  4. Repeating motions that may include the wrists, hands or fingers.
  5. Communicating with others to exchange information.
  6. Withstanding outdoor elements such as temperature, participation, and wind.

WORKING ENVIRONMENT: The environment is a typical office environment, school classroom, or home environment. While on location, work is generally performed indoors with ambient room temperatures, lighting, and traditional office equipment. This position will require the use of a company vehicle for business and requires travel to multiple worksites including other offices, residences, and various places around the community. Case Managers may be exposed to all low socio-economic homes as well as aggressive situations.

SALARY RANGE FOR THIS POSITION: Salary commensurate with experience and Kanza MHGC salary structure.
