Case Manager I - Emergency Men's Shelter Join to apply for the Case Manager I - Emergency Men's Shelter role at Catholic Charities of Southern Nevada Position Summary As part of a multidisciplinary Residential Services Team, the Case Manager I roles are to create initial contact and rapport with emergency shelter clients, facilitate shelter enrollment and diversion services, and coordinate and participate in the development and delivery of comprehensive Housing First Individual Service Plans. Case Managers I facilitate housing assessments, coordinate programs and services, provide supportive services using Housing First and Harm Reduction modalities, and play a crucial role in helping clients transition from homelessness to home. They advocate for clients, track outcomes, complete individual progress notes, and provide “under‑the‑hood” support in medical, mental, dental and transportation services. Essential Responsibilities Assess and identify client eligibility for community programs such as housing, employment, substance use, and nutritional assistance, and provide appropriate program guidelines. Work closely with Case Manager II’s to create a seamless transition from the homeless shelter to housing in accordance with clients’ Housing First Individual Service Plans. Provide case management services within the emergency men’s shelter. Facilitate night shelter enrollment and shelter diversion services. Maintain awareness of potential community resources and make proper referrals. Complete required program documentation using computer programs and data collection systems. Utilize electronic records to document and track all case management services and referrals. Assist clients in reducing system barriers and navigating the social services network. Protect confidential information, company property, and electronic data; comply with safety rules. Perform other related duties as directed. Knowledge, Skills & Abilities Computer literate with effective written and verbal communication skills. Ability to work independently or as part of a team. Task‑oriented, non‑judgmental, with personal boundaries and a strong focus. Respect for confidentiality involving both clients and employees. Culturally sensitive and able to work with diverse groups. Strong time‑management and prioritization skills to meet deadlines. Qualifications One year of full‑time, professional‑level casework experience in a social‑service agency preferred. Prior experience with homeless populations required. Experience with chronically homeless men preferred. Flexible schedule to work evenings and weekends is required. Physical Requirements Primarily works in a shelter dorm/office environment but also works throughout the agency and off site. Regular communication with clients, co‑workers, and supervisors. Operational use of office equipment, especially a computer, keyboard, and mouse. Ability to sit and/or stand regularly; walk continuously if needed; climb stairs as needed. May occasionally lift up to 25 pounds. This position pays $17.00 per hour.
