If so, Oakwood Creative Cares Adult Day Health Program is looking for a Care Navigator to be a trusted guide, connecting individuals to the best care, support, and services available.
Whats This Role All About?
Think of this as being a lifeline for families facing dementia. Youll provide personalized support, guidance, and advocacy to ensure individuals living with dementia receive the care they need while empowering their families with education and resources.
Using an evidence-based care model , youll work closely with members, families, and caregivers to create individualized care plans that promote independence, dignity, and quality of life . This role is perfect for someone who is both compassionate and strategic , blending emotional support with practical solutions to help families thrive.
What Youll Be Doing:
Guiding Families Through Dementia Care Develop personalized care plans that support the unique needs of each individual.
Assessing & Advocating Conduct comprehensive evaluations to ensure members receive the right care, at the right time.
Problem-Solving Like a Pro Help families navigate stressful situations, tackle challenges, and adapt to changes with confidence.
Educating & Empowering Provide essential dementia-related education, tools, and resources to families and caregivers.
Providing Emotional Support Be the go-to person for reassurance, guidance, and practical solutions.
Connecting to the Community Link members and families to support groups, social services, and engaging activities.
Tracking Success & Impact Collect and manage data to evaluate care outcomes and ensure quality services.
Sharing Inspiring Stories Work with our Creative Director to highlight member success stories and promote community engagement.

What Youll Need:
Bachelors degree Preferred
Experience in dementia care, care coordination, or patient advocacy
Proficiency in electronic health records (EHR) and care management software is preferred
CPR & First Aid Certified
Negative TB Test & Background Clearance
