Position Summary:
Investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
Reports to: Director of Care Management
FLSA: Exempt
Minimum Qualifications:
Minimum Education
Bachelor’s degree in Social Work required. Master's degree in Social Work preferred
Required Skills
Experience in Acute Care is preferred
Required Skills
Certifications:
Basic Life Support (BCLS) preferred
Required Licenses
[ Arizona, United States ] Social Worker
Licensed Clinical Social Worker in Arizona preferred
