Title: Agency Coordinator - San Ramon
Schedule: Full Time - 40 hours per week.
Must work weekends . (Fri - Tue)
Location: San Ramon, CA (onsite, no remote)

Position Summary: - Provide office support to the agency team - Administrator, Director of Patient Care Services, Clinical Managers, other Agency and Field Staff not limited to the following:
- Uploading additional patient information
- Process Provider orders
- Process visit authorization
- Supply disbursement
- Clinician schedules
- Agency coordination
- Answer and coordinate phone calls and messaging.
- Ensure appropriate interdepartmental communications to support essential agency activities.
- Ensures patient referrals are processed timely and per policy.
- Establishes and maintains positive working relationships with clients and team members.
- Supports the agency in the seamless transition of patients to home care by providing essential documentation and coordinates with multiple service providers.
- Ensures maximum third-party reimbursement through direct oversight of insurance verification and authorization processes.
- Provides feedback to the agency, including identifying opportunities to improve services to meet customer needs.
- Maintains comprehensive working knowledge of CVH Home Health Services' contractual relationships and ensures that patients receive services according to contract provisions.
- Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by CVH Home Health Services.
- Ensures compliance with State, Federal, and Joint Commission regulations and clients' contract requirements.
- Other duties as assigned to support agency projects and initiatives.
- Minimum Qualifications
- Minimum of 2 yrs. experience in a medical setting in either a patient care or administrative role preferred.
- Knowledge of MS Office, Google Suite, Internet Explorer, and professional telephone protocol.
- Basic knowledge of medical terminology is preferred.
- Able to deliver effective results and meet tight deadlines and targets.
- Excellent customer service and communication/interpersonal skills.
- Ability to work with high precision, paying attention to detail to achieve the high accuracy levels required and timely follow-up and close-out of all matters.
- Ability to communicate orally and in writing to minimize confusion and additional concerns.
